We will work with other relevant professionals and key people in your life to gain a holistic picture of your life
We will introduce you to your service manager and mentor your new support workers to ensure a consistent and high standard from the start
Can offer you support and advice to find new groups or activities, liaise with relevant housing associations or build towards specific goals
We will use this information to develop a person centred support plan and match you with a team of support workers based on your individual needs and requirements
